There is no additional cost of SMO service for our valued clients. The service is offered with no extra cost to the valued customers of Takaful Emarat.

Step 1: Download the SMO Authorization Form here.
Step 2: Fill-in the Authorization Form.
Step 3: Send the email along with the Authorization Form and soft copy of report/s to smo@takafulemarat.com.

Requirements: (All the following documents should be in English language only or translated to English language)

• Medical Reports which clearly indicate your medical history and current medical condition/s.
• Laboratory Investigation Reports.
• Radiology Investigation reports (in case of X-Ray, MRI or CT scan, original films are required in soft copy).
• List of prescribed medicines & current treatment plan.
• Any other document to help in clear identification of the medical condition/s.

Special Note:

1. The service is available as per the terms and conditions of the Takaful Plans.
2. Takaful Emarat retains the right of refusing any SMO request.
3. The second medical opinion will be available within 2 weeks; however it can take longer if additional information or laboratory results are required or if the most appropriate specialists are not immediately available.
4. Takaful Emarat carefully protects the confidentiality of your personal medical information and would not release this information to third parties for any purpose other than to provide you with the second medical opinion.
